FQMatch, Inc. ("FQMatch," "we," "our," or "us") is committed to protecting the privacy of our users. This Privacy Policy describes how we collect, use, store, share, and protect information submitted through the FQMatch platform and website (fqmatch.com) (the "Platform").
By using the Platform or submitting information through our registration forms, you acknowledge that you have read and understood this Privacy Policy.
2. Information We Collect
2.1 Information You Provide Directly
When you register or complete our intake form, we collect:
- Identity information: first name, last name;
- Contact information: email address, phone number;
- Location information: current city, state, or country of residence;
- Immigration and work authorization status: visa type (U.S. Citizen, Green Card, J-1, H-1B, EAD, or other);
- Medical training information: country of medical training, medical school graduation year, specialty;
- Licensing information: USMLE Step 1, 2, and 3 status; ECFMG certification status;
- Match history: NRMP Match participation;
- Professional experience: clinical experience, other relevant work history; and
- Preferences: state practice preferences, earliest available start date.
2.2 Information Collected Automatically
When you access the Platform, we may automatically collect:
- Device and browser information;
- IP address and general geographic location;
- Usage data, including pages visited and features accessed; and
- Cookies and similar tracking technologies.
3. How We Use Your Information
We use the information we collect for the following purposes:
- To assess your eligibility for alternative U.S. physician licensure pathways;
- To match your profile with FQHC and safety-net healthcare employers;
- To communicate with you regarding your application, match status, and platform updates;
- To improve and develop the Platform's intelligence and compliance capabilities;
- To generate de-identified, aggregated analytics for use with investors, FQHC partners, and policymakers; and
- To comply with applicable legal obligations.
We will NOT use your individually identifiable information for advertising, sale to third-party data brokers, or any purpose materially inconsistent with the purposes stated above.
4. How We Share Your Information
4.1 With FQHC and Healthcare Partners
FQMatch may share aggregated, de-identified, and anonymized cohort-level data with FQHC partners, health systems, and investors. Examples of shared data include: distribution of physician specialties in our applicant pool, geographic preferences, general visa status categories, and ECFMG certification rates. No individually identifiable information — including name, contact information, or unique credential identifiers — will be shared without your explicit written consent.
4.2 With Investors
We may share high-level, de-identified data with current and prospective investors as part of business reporting. This data will be aggregated and will not identify any individual user.
4.3 Service Providers
We may share your information with trusted third-party service providers who assist in operating the Platform (e.g., cloud hosting, email delivery, analytics). These providers are contractually obligated to protect your information and may not use it for independent purposes.
4.4 Legal Requirements
We may disclose your information if required by applicable law, regulation, legal process, or governmental request, or to protect the rights, property, or safety of FQMatch, our users, or others.
5. California Privacy Rights (CCPA/CPRA)
If you are a California resident, you have the following rights under the California Consumer Privacy Act (CCPA) and California Privacy Rights Act (CPRA):
- Right to Know: You have the right to request disclosure of the categories and specific pieces of personal information we have collected about you, the sources of that information, the purposes for which it is used, and the categories of third parties with whom it is shared.
- Right to Delete: You have the right to request deletion of your personal information, subject to certain exceptions.
- Right to Correct: You have the right to request correction of inaccurate personal information we hold about you.
- Right to Opt-Out of Sale or Sharing: FQMatch does not sell personal information as defined under the CCPA. We do not share personal information for cross-context behavioral advertising.
- Right to Limit Use of Sensitive Personal Information: To the extent we collect sensitive personal information (such as immigration status), you have the right to limit its use to purposes necessary to provide the service.
- Right to Non-Discrimination: We will not discriminate against you for exercising your privacy rights.

6. Rights for Users in Other States
FQMatch is committed to honoring privacy rights under applicable state laws, including but not limited to Virginia (VCDPA), Colorado (CPA), Connecticut (CTDPA), Texas (TDPSA), and other enacted state privacy frameworks. 7. Data Retention
We retain your personal information for as long as your account is active, or as necessary to fulfill the purposes described in this Privacy Policy, comply with legal obligations, resolve disputes, and enforce our agreements. You may request deletion of your data at any time by contacting help@fqmatch.com.
8. Data Security
FQMatch implements reasonable administrative, technical, and physical safeguards designed to protect your information from unauthorized access, use, disclosure, or destruction. However, no method of transmission over the internet or electronic storage is completely secure. We cannot guarantee absolute security of your information.
9. International Users
The Platform is operated from the United States. If you are accessing the Platform from outside the United States, please be aware that your information may be transferred to, stored, and processed in the United States, where data protection laws may differ from those in your country. By using the Platform, you consent to such transfer and processing.
10. Children's Privacy
The Platform is not directed at individuals under the age of 18. We do not knowingly collect personal information from minors. If we become aware that we have inadvertently collected information from a person under 18, we will promptly delete such information.
11. Cookies and Tracking
We use cookies and similar tracking technologies to operate and improve the Platform. You may configure your browser to refuse cookies; however, some features of the Platform may not function properly without them. We do not currently respond to "Do Not Track" signals, but we will update this policy if our practices change.

We may update this Privacy Policy from time to time. We will notify you of material changes by email or by posting a prominent notice on the Platform. Your continued use of the Platform following notice of changes constitutes your acceptance of the updated Privacy Policy.
